Where Are Those Blasted Keys?!
The original lookout on the Titanic was replaced with a more experienced one, who never got the keys to the locker which contained the ship’s only pair of binoculars. Naturally, no one saw the iceberg waiting to be struck until the ship was too close to be steered away from the obstacle.
What Was The Mistake?
The Mistake here was the fact that the lookout on the Titanic was not organized, and couldn’t do his job properly due to the former lookout being careless.

The mistake was a Careless mistake, and possibly a lack of knowledge, because the former lookout may not have known that there was a special set of keys to the locker that contained the keys.
What Were The Consequences?
The consequences were that the Titanic sank, and many people died, astonishingly, the lookout himself survived the disaster.
What Did People Learn?
People most likely learnt to be more careful, and always be organized, because if the former lookout hadn’t been so careless, hundreds upon hundreds on lives could’ve been saved.
Has It Affected The World?
I think that it may have affected the world, because this could be the reason why ships now have radars and scanners for the ocean around them.

Wow, has it already been 30 years? I remember boarding that ginormous ship, I remember saying to myself: “This is the trip of a lifetime!” My family, which consisted of me, my mom, my dad, and my nana, all ogt on, and prepared for what we thought would be a fabulous day. The memories of the day the iceberg was struck still haunts me, the scraping noise, then the feeling of the boat starting to tip over, I asked “What happened?” My mother was surely shocked as well, she said “Water’s probably gotten into the ship.” I remember scrambling to get on the lifeboats, and seeing the rest of my family stuck on the sinking behemoth, I spent the hours crying, and never saw my beloved family ever again. That day, 30 years ago, still brings me close to tears, whenever I think about it.

Why Didn’t You Learn Your Directions?!
Ferdinand’s driver -Leopold Lojka was driving Franz Ferdinand and his wife when he took a wrong turn, which Ferdinand straight to his assassination - Gavrilo Princip, who shot both the passengers. This accident is the suspected cause for the beginning of World War one.
What Was The Mistake?
The mistake was on the drivers part, because he accidentally took the wrong turn, and drove Franz Ferdinand to his assassination, who shot both his wife and him.

I think that the mistake was a careless mistake, because the driver may not have been paying too much attention to his duty. It could also have been a lack of knowledge, because the driver may not have known where he was supposed to go.
What Were The Consequences?
The consequences of the mistake were that Franz Ferdinand died, and so did his wife. The driver probably earned a terrible reputation afterwards.
What Did People Learn?
I don’t really think that anyone learnt from this scenario, unless it is for teachers to educate students about.
Has It Affected The World?
It definitely has changed the world, because this could be the reason why World War 1 started in the first place. Followed on by World War 2.

I remember saying to myself: “There once was a time to go back, you just missed it.” Franz Ferdinand, Archduke of Austria-Hungary, heir to the throne, was shot and killed right before my very eyes, even with hundreds of people watching, the assassinator still murdered both of them, Ferdinand and his wife. What will happen after? Will he start shooting us? I start running back, in hopes of getting the story around, I was still little back then, and loved to turn everything into a competition, now, 63 years old, I look back at that experience and think to myself, wow, I was there at the tragedy that started World War 1.

Didn’t You Know Oil Isn’t Good For The Fishies?!
On April 20, 2010, the British Petroleum Oil Rig Burst, scattering nearly 5 million barrels of oil from the well. It was the largest oil spill, passing the oil spill that happened in 1979 in the Bay of Campeche. The British Petroleum spill affected approximately 68,000 square miles.
What Was The Mistake?
There were apparently 8 reasons why the BP Gulf Oil Spill occured, so I will name 2 of them, the leak wasn’t spotted soon enough, there had previously been a leak in the oil rig, but it had not been spotted until it was far too late. There was also no gas alarm installed in the area, so no one was warned of the disaster waiting to happen.

The mistake was a lack of knowledge because the workers didn’t know about the leak, and also a careless mistake, because nobody decided to install a gas alarm.
What Were The Consequences?
The consequences of the careless mistake were that 11 people died, and 4.9 million barrels of oil into the sea, intoxicating heaps of marine life.
What Did People Learn?
I think people possibly learnt that oil rigs require a lot of attention, and that there should always be someone responsible on the site.
Has It Affected The World?
I think that is has affected the world, because many people had to help out with the clean up, and people who relied completely on fish and lived around that area probably had a hard time finding food.
Memoir
It’s already been 60 years, since I saw that humongous explosion, and read all the news articles, My mind has been scarred by that experience, I remember asking my mom: “Mommy, what are those people doing?” My mother’s eyes were filled with tears, “That’s where daddy is love. Some oil’s just spilled.” The memory has stayed with me the entire time, and I continue to think about it. The people are still cleaning it up. Everyone around including myself relies on fish as our main diet. I can't believe we actually survived through that. Dad, who died in the explosion, will continue to have our deepest respect.
